The package includes a comprehensive SOP which, if followed, means you should complete the survey within two weeks.
Paper Based Reports
Your Results will contain:
1) A coloured graphical summary of your returns
2) An analysis of the graphs by one of our directors
3) An action plan form which you can complete and show to your PCT
4) A colour poster for display, showing results & comments
5) Around 3 months after the year end, you will get a benchmark report.
Extra Question
You can have an extra question, which will allow your customers to say whether they are interested or not in 16 different services, with a box for any other suggestions. This will help you provide evidence of services your customers are interested in, so you can approach the PCT(LHB) or PBC for funding or to start the service on a fee paying basis.
There is no charge for printing the extra question but there will be a small cost to summarise of only £5.00 + VAT per 50 questionnaires.
This Survey MUST be carried out between 1st April 2010 and 31st March 2011
Don't Forget!! The later you return your completed surveys for return, the longer your report will take to get to you!!
