Once ordered you will receive a package containing 175 personalised, colour printed questionnaires in A5 booklet format, 125 of which is your contractual requirement. You will also receive two large copy questionnaires for photocopying, a ballot box label (you provide your own ballot box), an SOP which if followed, you can complete your survey in approximately 15 working days and a stamped addresses jiffy bag for you to return you required amount to us. On completion, you will receive a 7-10 page report containing written and graphical analysis, a comments page, an A4 poster for you to hang in your pharmacy and an action plan. It is stated in the contract that on visiting, the PCT may ask for your completed action plan - it says nothing about providing them with a copy of your full report.
Please ensure your pharmacy name and address is spelt correctly when ordering as this is the address used on your questionnaires.
