The package includes a comprehensive SOP which, if followed, means you should complete the survey within two weeks.
Paper Based Reports
Your Results will contain:
1) A coloured graphical summary of your returns
2) An analysis of the graphs by one of our directors
3) An action plan form which you can complete and show to your PCT
4) A colour poster for display, showing results & comments
If you need extra questionnaires at any time or extra copies of your report - we won't charge a penny!!
This Survey MUST be carried out between 1st April 2012 and 31st March 2013
Don't Forget!! The later you return your completed surveys for report, the longer your report will take to get to you!! This is why we offer a 20% rebate for those who manage to return their completed questionnaires to us by 30th September 2012 or a 10% rebate by 31st December 2012.
